Home Health RN

Responsibilities:

• Assess patient to determine skilled home health care needs
• Develop a Plan of Care to address patient’s needs
• Re-assessments and changes to Plan of Care as necessary
• Supervision of caregiver staff assigned to the patient
• Patient and family education for safety, best care outcomes, and to prevent hospital re-admissions
• Develop discharge planning needs when goals have been met

Requirements:

• Valid & current RN license
• Skilled Home Health and OASIS experience preferred
• Acute Care experience preferred
• Ability to learn EMR software
